The Center for Managing Chronic Disease works to build the capacity for effective chronic disease prevention and management. The Center accomplishes this by putting people at the center of solutions. The Center brings together national and international experts in chronic conditions—heart disease, respiratory disease, epilepsy, diabetes and others—who are doing innovative work related to the social, behavioral, environmental and clinical aspects of disease management. Vulnerable populations are offered programs that encourage them to take control of their chronic condition and enjoy a better quality of life.
